Physicians

As pulmonologists, our physicians possess specialized knowledge and skill in the diagnosis and treatment of pulmonary (lung) conditions and diseases. Pulmonology is classified as an internal medicine subspecialty. Because pulmonologists encounter a variety of clinical problems, they must have knowledge of internal medicine and other specialties in order to obtain certification. Pulmonologists may serve as consultants to other physicians as well as provide long-term care to patients with chronic pulmonary conditions. Our pulmonary physicians also serve as critical care consultants.

Our physicians perform Bronchial Thermoplasty, a new treatment for moderately severe asthma. We were one of only 11 sites in the country to participate in the Clinical Trial and were the first center to perform the procedure after it was approved by the FDA. Our group is actively involved in clinical research studies.

As a service to the community, all of our physicians volunteer at the Arlington Free Clinic.

Our physicians are certified by the American Board of Internal Medicine (ABIM). Our Physician Assistants are all board certified by the National Commission on Certification of Physician Assistants.


Steven M. Zimmet, M.D., F.A.C.P., F.C.C.P.

Dr. Zimmet received his undergraduate and medical degrees from the University of Virginia. He completed his postgraduate training in internal medicine and pulmonary medicine at Georgetown University Hospital. He is currently a Clinical Professor of Medicine at Georgetown University and is also an Associate Clinical Professor of Medicine at George Washington University. He is a Fellow of the American College of Physicians and the American College of Chest Physicians and has served as president of NAMDRC, a national association of pulmonary physicians. Dr. Zimmet is past president of the Virginia Thoracic Society, the American Lung Association of Northern Virginia, and the Virginia Society of Internal Medicine. He is Medical Director of the ICU and Respiratory Care Department at Virginia Hospital Center, and serves as a member of the Board of Directors of the hospital. A native of Arlington, Dr. Zimmet joined PMA in 1974.

M. Anthony Casolaro, M.D., F.A.C.P.

Dr. Casolaro received his undergraduate degree from the University of Virginia and his medical degree from Georgetown University. He completed his internal medicine training at GradyMemorial Hospital and Emory University. He received his pulmonary medicine training as a recipient of a National Research Service Award at the National Institutes of Health. Subsequently hewas Clinical Chief of the Pulmonary Branch at the National Institutes of Health. He is a Fellow of the American College of Physicians and is a Clinical Associate Professor of Medicine atGeorgetown University. He authored numerous papers and textbook chapters in the field of pulmonary medicine. Dr. Casolaro serves as Vice President of the Medical Staff at Virginia Hospital Center. He is currently team physician for the Washington Redskins. Dr. Casolaro joined PMA in 1987.

Lawrence M. Stein, M.D., F.A.C.P., F.C.C.P., F.A.A.S.M.

Dr. Stein received his undergraduate degree from Fordham University and his medical degree from the State University of New York. He did postgraduate training in internal medicine at Columbia University College of Physicians and Surgeons and pulmonary medicine training at the Albert Einstein College of Medicine. Dr. Stein is board certified in internal medicine, pulmonary medicine, critical care medicine, and sleep medicine. He is a Fellow of the American College of Physicians and the American Association of Sleep Medicine. In addition, he is a Clinical Assistant Professor of Medicine at Georgetown University. Dr. Stein has expertise in sleep medicine and pulmonary rehabilitation and is Medical Director of the Sleep Laboratory and the Acute Pulmonary Rehabilitation Unit at Virginia Hospital Center. He joined PMA in 1991.

David R. Duhamel, M.D., F.C.C.P.

Dr. Duhamel received his undergraduate and medical degrees from Georgetown University. He also completed his internship and residency in internal medicine and served as Chief Resident atGeorgetown. He trained in pulmonary, critical care, and interventional bronchoscopy at the University of California, San Diego. Dr. Duhamel has expertise in diseases of the airway and their management including YAG laser therapy, bronchial stenting, and brachytherapy. He is Director of the Pulmonary Special Procedures Unit and Lung Cancer Center at Virginia Hospital Center. He alsohas an interest in the diagnosis and management of pulmonary hypertension. He has authored several articles and book chapters in pulmonary medicine. He is co-author of The Atlas of Diseases of the Airway. He joined PMA in 2002.

Jeff B. Hales, M.D., F.C.C.P.

Dr. Hales received his medical degree from Vanderbilt University where he trained in medicine and was Chief Resident. He received his undergraduate degree from Duke University. He did pulmonaryand critical care training at Johns Hopkins University School of Medicine. Dr. Hales is board certified in internal, pulmonary, and critical care medicine. He has extensive experience in multimodality management of patients with lung cancer, especially in diagnostic and staging techniques. He is a member of the American College of Chest Physicians, the American Thoracic Society, and the American Association for Bronchology. Dr. Hales is Chief of the PulmonaryDivision at Virginia Hospital Center. He joined PMA in 2003.

Christopher C. Wyckoff, M.D., F.C.C.P.

Dr. Wyckoff graduated summa cum laude from the University of Rochester where he went on to receive his medical degree. He completed his internal medicine residency, served as Chief Resident, and completed the pulmonary and critical care fellowship at Georgetown University. Dr. Wyckoff is board certified in internal medicine and pulmonary medicine . He has received multiple teaching awards, including the "String of Pearls" teaching award given by the medical students and the Paul Wilner Award from the residents for excellence in teaching among the fellows in internal medicine. He is also a member of the AOA medical honor society. He has done extensive research in management of malignant pleural effusions, performing biopsies of mediastinal/hilar lymph nodes using innovative methods, as well as sleep apnea screening. Dr. Wyckoff joined PMA in 2008.

Mary-Margaret Lewis, M.D., F.C.C.P.

Dr. Lewis received her undergraduate degree from Boston College and medical degree from Georgetown University School of Medicine. She completed her residency in Internal Medicine at Georgetown University Hospital where she remained to complete her fellowship training in Pulmonary, Critical Care and Sleep Medicine. She is Board Certified in Pulmonary Medicine, Sleep Medicine and Critical Care Medicine, a fellow of the American College of Chest Physicians, a diplomat of the American Academy of Sleep Medicine, and a member of the Society of Critical Care Medicine. In her previous position at the Washington Hospital Center she won the Nurses' Choice Physician Collaboration Award. Dr. Lewis has a special interest in Sleep Medicine and also has extensive experience in Critical Care Medicine. Dr. Lewis enjoys the role of physician-educator as it applies to teaching physicians-in-training as well as her patients. Dr Lewis had spent a year with PMA at the beginning of her medical career and now rejoins PMA in 2010.

John K. Kilcullen, M.D.

Dr. Kilcullen received his undergraduate degree from Hampshire College and his medical degree from Albert Einstein College of Medicine. He completed his residency in Internal Medicine at Montefiore Medical Center where he stayed for a fellowship in Critical Care Medicine. He worked previously in New York City; Washington, DC; Daytona Beach, Florida and has joined PMA as a full-time ICU physician at the Virginia Hospital Center. Before his medical career, Dr. Kilcullen worked as a civil rights lawyer, followed by three years in West Africa working in international development. He has published in the fields of law, bioethics and public policy. With his PMA colleagues, he shares teaching responsibilities of medical students and resident physicians from Georgetown University. Dr. Kilcullen has been with PMA since January 2011 and is a Hospital Intensivist. This means he works in the hospital only doing Critical Care medicine.

Kari Esbensen, M.D.

Dr. Esbensen received her undergraduate degrees in biology and theology from the University of Notre Dame before moving to Northern Virginia to pursue her studies at Georgetown University, where she earned both her medical degree and a PhD in Philosophy with a concentration in Bioethics. She completed her internship and residency in internal medicine at Georgetown University Hospital. Her special interests include the collaborative management of diabetes and other chronic medical conditions, preventive health care, smoking cessation, end-of-life decision making, bioethics and health policy, and teaching medical students and physicians-in-training. Dr. Esbensen joined PMA in 2011.

Amit V. Patel, M.D.

Dr. Patel received both his undergraduate and medical degrees from Northwestern University as a member of the Honors Program in Medical Education. He completed his Internal Medicine residency at Georgetown University and subsequently completed his fellowships in Pulmonary, Critical Care and Sleep Medicine at New York University. After finishing his training, Dr. Patel stayed in New York and joined the Weill Cornell Medical College faculty as an Assistant Professor of Medicine attending in the Center for Sleep Medicine, Pulmonary Service and Medical and Neurosurgical Intensive Care Units. Dr.Patel has a special interest in Sleep Medicine and has published numerous papers related to the physiology and treatments of obstructive sleep apnea (OSA) including new, non-CPAP, therapies for OSA such as the ProventR Nasal Resistor Valve. He also has experience in dealing with a wide variety of sleep disorders including insomnia, narcolepsy, restless legs syndrome and sleep walking. Dr. Patel returned to the Washington DC area to focus on clinical practice and to be closer to family. He joined PMA in September 2011.

Michael D. Jacobson, M.S.H.S., PA-C

Mr. Jacobson received his medical training at the George Washington University School of Medicine and Health Sciences. He received his undergraduate degree from the University of Rochester, where he also served as a critical care E.M.T. Mr. Jacobson is board certified as a physician assistant by the National Commission on Certification of Physician Assistants. He is a member of the American Academy of Physician Assistants. He has a special interest in internal medicine andworks closely with all of the physicians at PMA. He is an Assistant Clinical Professor of Health Care Sciences at the George Washington University School of Medicine and Health Sciences. He joined PMA in2005.

Lisa C. Moak, M.P.A.S., PA-C

Ms. Moak received her medical training at the James Madison University College of Health and Sciences. She received her undergraduate degree from the University of Virginia where she graduated with distinction. While at the University of Virginia, she was a research assistant working towards the development of a vaccine for melanoma. She is a member of the American Academy of PhysicianAssistants. Ms. Moak is board certified as a physician assistant by the National Commission on Certification of Physician Assistants. She has a special interest in women's health and endocrinology and works closely with all of the physicians at PMA. She is an Assistant Clinical Professor of Health Care Sciences at the George Washington University School of Medicine and Health Sciences. She joined PMA in 2007.

Erin Chaffee, MSHS, PA-C

Ms. Chaffee received her medical training at the George Washington University School of Medicine and Health Sciences. She received her undergraduate degree from the University of Massachusetts where she graduated with distinction. Ms. Chaffee is board certified as a physician assistant by the National Commission on Certification of Physician Assistants. She is a member of the American Academy of Physician Assistants. She has a special interest in women’s health and internal medicine and works closely with all of the physicians at PMA. She joined PMA in 2010.

David Glasser, PA-C

David Glasser received his medical training at the Long Island University/Brooklyn Hospital PA program in New York City. He graduated Magna Cum Laude and has received multiple clinical and academic awards. His training centered around Emergency Medicine and Critical Care Medicine, and he was a key member of the emergency response team to Ground Zero on 9/11 and the subsequent treatment of patients. He has had experience as a Hospitalist. He is a Fellow of the American Academy of Physician Assistants, and currently is a Visiting Professor at the George Washington University Physician Assistant Program. His interests include: primary care medicine, infectious disease medicine, and asthma management. He joined PMA in 2010.
