Missed Appointment/Cancellation Policy
Here at Pulmonary & Medical Associates we truly value our relationship with our patients, and appreciate that you have chosen us for your health care needs. We are committed to each and every patient, and in order to ensure timely access to high quality healthcare, the following changes to our office policies have been made:
For patients with insurance, PMA will submit all claims to the appropriate insurance carrier. All co-pay amounts will be collected at time of service as required by your insurance contract. For patients without insurance, PMA requires that payment for all services be made at the time of your visit. Our doctors are on staff at the following medical facilities: Be sure to obtain the results of all your tests. Laboratory results may usually be obtained one week after your visit by calling our patient laboratory information number at 1.800.400.0470 and enter our office code 2175 followed by the # key. After PMA is verified, touch 1 and enter your medical record/account number followed by the # key. This will connect you with a message from your physician with your laboratory results. You can call 24 hours a day, 7 days a week. One of the ways you can help guide your treatment is by bringing a list of medications (including dosage and strength) with you at the time of each visit with the doctor. Having the telephone number of your pharmacy on hand is also helpful. PMA physicians will not grant refills over the weekend or after hours. Please ensure that you have a sufficient quantity of the medications you need on hand, prior to the weekend. A doctor from our group is available on call 24-hours a day for emergencies. If an emergent problem arises after hours, please dial 703.521.6662 to reach the physician on-call. Please remove any "caller-ID" blocks on your phone to allow us to return your call. In a true emergency, go the nearest emergency room. Once you have arrived, please have the emergency room notify our office. PMA fosters an atmosphere of mutual respect and good communication among our patients and staff alike. Honest and open communication facilitates excellence in care. Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients and a copy is available upon request. |
